Termination of GP Medical Cover in Oxon Community Hospitals

CHO has since explained  to me, its views on the previous bed fund arrangements and whether redundancy applies

With the bed fund existing for decades, and so many predecessor organisations, finding complete paperwork has been difficult, but CHO has attempted to do so.

All facts discovered were given to a lawyer for an opinion on the nature of the historic contract arrangements and this has informed the CHO view  below.

CHO believes that in most cases the bed fund was a contract for services, and redundancy does not  apply.

With the knowledge you have of your own documentation, you may reach a different  conclusion.

Dear Colleagues
I am organising an informal meeting at GreenCollege, Woodstock Road, Oxford for  clinicians who work at the OXEMS service or who are interested in the provision  of OOH care for their patients in the OxfordCity area. (Anyone with an  interest in urgent care from the wider Oxfordshire area would, of course, also be welcome).
Laurence Leaver has kindly booked a room at Green College  which is reasonably central, accessible by Park and Ride and has a fair bit of  nearby on-street pay and display parking. We will get some wine / beer / juice /  nibbles available to keep us going and will appreciate a small donation towards  the cost.

The idea is to spend about an hour or so from 1900-2000  discussing the service and any concerns / problems  / suggestions that you may  have about workload / organisation / sustainability / anything else. We will then decide how to take these forward, perhaps by establishing a forum.....
Dr Ken Mann is aware that we are meeting and was supportive  when I mooted it, being aware that OXEMS clinicians' input into the clinical management team is more or less non-existent at the moment.
I hope that  you will come along and share your views with (hopefully) like-minded  colleagues. Could you email a quick RSVP or apology so I can have some idea of numbers for catering / room size etc? How to calculate your likely income from the Oxon Diabetes LES  
                                                                                                         (08/01/10) 

The attached automated Excel calculator allows you to put your K number into the pink cell of  the sheet “income possibilities”, and then get all your possible income levels from the 2 components of the Oxon Diabetes LES in the orange cells of the same sheet.  

This may allow you to make a decision on whether the income is sufficient for the work involved especially given the obligatory training  described in LES (see text of the LES attached and Sarah McGuinness’s email  below) 

For both components of the LES you will need to judge on past performance which tier of achievement you are likely to reach then read across to the associated income under the LES

  • n  accreditation panel will then meet to assess the applications on 13th January 2010 withfeedback being given to practices between 25th and 29th January 2010.
  • Training on  Personalised Care Planning is offered on 18th February or 9th March 2010 from9:30 to 16:30, details to  follow.
  • The  enhanced service commences 01st April 2010.

New version of LMC statement on MSK hub to go to practices  
                                                                                                         (20/10/09)

On 14.10.09, LMC and practices were told by the PCT that an obligatory MSK hub was being introduced and this would operate from 19.10.09

The MSK hub is being introduced because current referral levels to MSK secondary care are unaffordable by the PCT.

LMC finds the arrangements and the timescale unacceptable and will be asking the PCT to alter its plans.

From Monday 19th October, GPs must refer first to a MSK hub that will assess the referral indication.  Not one of the  assessors is a GP.

If approval for onward NOC referral is given the hub will contact the GP with a reference number. The GP then has to send a proper referral letter containing this number via C+B.

The duplication in GP work is obvious.

From the patient’s point of view, making the system more complicated will undoubtedly produce administrative errors resulting in referrals disappearing into the ether and possible harm

LMC suggested that the usual referral letter be sent to the MSK hub that would either pass it to the NOC or divert it to another provider such as an MSK GPsSI or physio. The PCT has said that the Hub cannot operate C+B which is obligatory, hence the need for a second letter from the GP.  

The LMC does not accept this position. If the PCT wants greater demand management, then the MSK hub (or the TVPCA Referral Management Centre) has to operate C+B or C+B has to be dispensed with for MSK conditions.

LMC plans to make the strongest representations to the PCT.

LMC letter about funding a Darzi Centre in a recession   (20/10/09)

The LMC plans to work  with the PCT so that the predicted reduced PCT funding is best used in a  recession.

However, this will not involve LMC colluding with the PCT in avoiding decisions that the government might disagree with.

Rick Godlee has  therefore written the attached letter to the PCT chair with LMC views on  renegotiating the Darzi centre contract in Banbury.

LMC concern is that the  centre is predicted to consume Ł1m per annum of PCT spend at a time when other  more important services are being cut.

Audit of childhood immunisations                                         (27/08/09)

Your views please on Oxon PCT giving this research team access to OPCT child vaccination  records

Explanation  and Analysis

Patients who have  undergone chemotherapy should apparently be revaccinated, but it appears this doesn’t always happen.

The Paediatric SHO named below is part of a research team looking at this issue and wants to look  at PCT child vaccination records (which should now match the records held by practices) to see if this record is different to the official child health record. 

I do not imagine such  access would be abused and would predict that neither patient not doctor would object to such analysis.

It seems to comply with  the information sharing document recently circulated by BBOLMC  (attached

The key part of this document is:
The Code permits practices to pass un-anonymised personal patient information to PCTs without individual patient consent when all 4 conditions below are met:

  • It is  impractical to obtain individual patient consent
  • The  information is needed for the PCT to perform its planning and commissioning function
  • The  likelihood of detriment to the patient from information disclosure is negligible
  • PCT use of the information is subject to a strictly enforced internal PCT confidentiality  policy

A while ago the PCT gave permission  to Dr Avishay Sarfatti, an SHO in paediatrics, to audit the re- immunisation of  children who had undergone chemotherapy. He has looked at the child health records where it appears that these children have not been re-immunised.  He would now like to look at the GP's records to see if anything different is recorded in their records.

As “Decision Support” have the immunisation data from General Practices for the purpose of the Improving Immunisation Project I am emailing to ask if you think that we should approach Practices  to ask if we can use the GP data to assist the audit, thereby saving  Practices the need to be involved?

The reason for this audit is directly for the benefit of patient care.

Following our telephone  conversation I am sending you this email to further explain the audit project we discussed.

I am a SHO in paediatrics  with the Oxford Deanery. I work with Dr Matthew Snape, a consultant paediatrician at the JR and the Oxford paediatric vaccine group, looking at the  re-immunization of children post chemotherapy.

The ORH trust's guidelines on the immunization schedule to be used with children who are immunodeficient  (including children who underwent chemotherapy or bone marrow transplant)  was recently updated in line with the RCPCH guidelines. We would like to take this opportunity and see whether these guidelines are in fact being followed in  the above group of children.Â

Past attempts to establish whether this subgroup is being re-immunized according to a modified schedule  have not been fully successful. This is because the immunisation of these  children is conducted by their general practitioner, and previous audit attempts  looked at hospital notes solely, where there is no mention of immunization status post treatment.

we took this a step further, and with the PCT's permission accessed the Child Health Computer to establish  the vaccinations each child had. When doing so, it appeared that most children  were not re-immunized once they completed their course of chemotherapy. We assume that this is not in fact the case, but rather a reflection of a delay  in reporting / data entering onto the child health  computer.

Our next step is to contact  GP surgeries directly, and ask for their permission to obtain information  regarding the immunization of these children. To that end we, of course, need the PCTs permission and assistance.

Please note the understanding of the Oxford Radcliffe Trust of its obligation to provide sick notes to both inpatients and outpatients.

“The rule now is that the hospital should provide sickness  certification for the full period for which we believe that the  patient should be off work following admission to hospital.
The two week rule was part of the previous guidance governing this topic and has been superseded by the more recent guidance above.”.

  • Talk about the pressure you are  experiencing with your colleagues, family, friends
  • Put your energies into trying to finding solutions, manage your time better, learn how to say no
  • Look at your work/life balance – is it the way you want it?
  • Become more aware of how pressure arises to avoid it becoming a problem
  • Make time to relax, eat healthily, take  more exercise, try to get more sleep.

I  believe you are already aware of the planned Keep Our NHS Public meeting in Banbury next Tuesday 8th July  at The People's Church, Horsefair, Banbury (opposite St. Mary's Church).
Attached is the leaflet we will be distributing in the town on Saturday.
Would you feel able to send it and a general notice about the  meeting out to your local GP colleagues? The meeting is part of a series KONP Oxfordshire are doing this year, moving around the county looking at local  issues. The issue of polyclinics being handed over to big business and creating  competition in primary care is another move in what we see as an orchestrated campaign in the NHS being an open market.
The plan for the evening would be to start with a 'drop in at 6.30pm'. We will do a short more formal  presentation at 7.30pm and then have an hour for open discussion. We would also be interested in discussion about how we can help assist locally in campaigning  about the polyclinic.
